INDEX BOOK and EXAMS: Students who do not have an index book can not enter the university and can not take midterm, final and make-up exams.
LOST INDEX BOOK: If you have forgotten your index book in your cuntry or if you have lost your index book, you should get a new one from Student Affairs Office according to related procedure.
OLD PLASTIC ID CARDS: Old plastic ID university cards are invalid according to macedonian law. Because of that all students should carry their indeks book with themselves every time. If they dont have it, they can not accepted to the midterm, final and make-up exams.
HOW TO CHANGE THE OLD INDEX NUMBER IN THE INDEX BOOK WITH A NEW ONE? 1.Students Affairs Officer can change the old index number in the index book with a new one. 2.A line should be drawn over the old index number and the new index number should be written with a red pen. 3.Students Affairs Officer should sign and stamp this change.
STUDY PROGRAM CHANGES: If a self financed student changes the study program within the same faculty, no changes will be made in his/her index book ID number. Self financed Student’s new study program should be written below the former study program. The former department name should be drawn with a red pen and signed by the Faculty Dean and stamped. If a self financed student changes his/her study program within the same faculty, no changes are made in the Main Registration Book.
REGISTRATION FORMS AND INDEX BOOKS: Preporatory program and First year "self financed students" can not change their study programs till at the end of second semester after signing and filling out 16 pages registration form. If they change their study programs in the index book later than signing their registration form this kind of changins are invalid.
ABUSING THE INDEX BOOK: Discipline decision was given about that student he has written wrong GRADE points in his index book and/or abused the university system.
FACULTY CHANGES AND INDEX BOOK
If a student changes study program from one faculty to another, his/her ID number in the Index book should be drawn with a red pen and next to it, the new ID number should be written with a red pen. Student’s new faculty and new study program should be written below the former ones. The former faculty and former study program should be drawn with a red pen and signed by both former faculty dean and new faculty dean.
DOUBLE PROGRAM STUDENTS AND INDEX BOOK:
A student who attends a double study program will get a new index book for the second study program. This student will have two different ID numbers. This student is registered twice (separately for each faculty) to the university. First year course grades will be written two times in their index book.
INDEX BOOK DELIVERING WITH SIGNATURE:
All students will take their index book in return for signature. (11-08-02_Index_Book_Submissions.doc).

Index book price is 100 denar.
